This book provides a comprehensive clinical review of Hypertrophic Cardiomyopathy (HCM), the most common genetic disorder of the heart characterized by dysfunctional contractility at the sarcomere level, resulting in the development of abnormal and occasionally focal hypertrophy on a macroscopic level. Editor, Srihari S. Naidu, has brought together a world renowned group of experts to review various different topics but, with a practical focus that will enable readers to establish the evidence-based best practice in any potential scenario. Treatment modalities including medications, pacemakers and defibrillators, and invasive septal reduction therapy (both surgical myectomy and alcohol septal ablation) will be discussed. Chapters on genetics, family screening, lifestyle concerns, and athletic screening have additionally been added given the ongoing controversies and differences of opinion on many of these issues. Each chapter within Hypertrophic Cardiomyopathy begins with key points of knowledge and ends with clinical pearls that have not previously been disseminated to the wider community. The practical approach of the entire book continues with dedicated chapters on creating a Center of Excellence, including how to facilitate the multi-disciplinary approach, and on case-based reviews and discussions allowing readers to further understand how to integrate the knowledge gained from each chapter into the comprehensive and longitudinal care of the individual patient and family. The last chapter takes the reader through the management of actual patients, showing over decades the nuances to diagnosis and management and the sometimes abrupt changes in the course of their diseases that necessitate correspondingly abrupt modifications in treatment. This book will be an essential text for Trainees, Fellows, Residents and board-certified physicians in cardiology, interventional cardiology, cardiac surgery, cardiac imaging, sports medicine, paediatric cardiology, genetics and genetic counselling, and electrophysiology.

Therapeutic Hypothermia After Cardiac Arrest

Therapeutic hypothermia has emerged as a very important treatment option for patients with cardiac arrest as it provides significant protection from developing neurologic injury once the patient has been successfully resuscitated. Studies have demonstrated over 15% absolute risk reduction in death and neurologic injury using this therapy. Although hospitals and medical centers have become familiar with this important intervention it still remains greatly under utilized due to an experience and lack of resources to safely and effectively deploy such a program. The objective of this book is to educate and familiarize both providers and institutions as to how to develop and deploy and provide therapeutic hypothermia to their patients. The current knowledge for this is provided by speakers and national experts and also by literature review. There are several courses being provided on this as well throughout the US. These are good venues for people to come and see and get hands on experience, but there still needs to be a concrete book with references on how to go about getting this program started.

End of Life Care in Cardiovascular Disease

End-of-life issues in cardiology are becoming increasingly important in the management of patients in the cardiac unit, but there is frequently a lack of understanding regarding their impact on cardiology practice. The cardiac unit is increasingly becoming the location whereby a number of key clinical decisions relating to end-of-life care are being made, such as the decision to remove medications, the appropriate removal of cardiac devices, the management of do not resuscitate orders and the requirement for other cardiac procedures in light of the management of the terminally ill cardiac patients. Those working in palliative care need input from the cardiovascular team as the cardiologist is frequently still managing these patients until they are moved to the hospice. That this move into a hospice is often delayed until the very last moment, there is considerable onus on the cardiovascular management of these patients to be much broader in scope and take account of some of the more palliative medical decisions needed in this group of patients. This concise reference will detail the practical issues open to cardiovascular physicians and those medical professionals who manage patients reaching the end of their life from a cardiology perspective. It will detail the full management options open to them to ensure that their practice is in line with the requirements of the patient nearing the end of their life whether the cause be cardiovascular in origin or who need appropriate management of secondary cardiovascular symptoms. It will also include the various ethical, cultural and geographical issues that need to be considered when managing these patients.

This volume discusses the many ways to prevent, identify and control hypertension in African Americans, a common and potent risk factor for virtually all forms of cardiovascular-renal diseases. Comprehensive chapters address modifiable risk factors, such as lifestyle changes, especially sodium restriction, and appropriate combination pharmacotherapy. Emerging devices and evidence-based approaches that may also enhance effective blood pressure control and decrease the disparate cardiovascular disease risks, including MI, stroke, HF, and cardiorenal metabolic syndrome and diabetes are also discussed in detail. Written by a wide-range of experts in the field, Hypertension in High Risk African Americans: Current Concepts, Evidence-based Therapeutics and Future Considerations is a valuable resource for clinicians, researchers, health administrators and public health policy leaders to better understand the best practices and unique aspects of risk assessment and treatment of hypertension and co-morbid conditions in African Americans.

This book provides an in depth review of the current practice in treating elderly patients with coronary artery disease (CAD) as well as an update of the existing literature. It is intended to reflect the current science of elderly patients with stable angina, acute coronary syndromes and those patients who are forwarded for CAGB procedures. Evidence in the elderly patient group is often limited and/or non-existent therefore the main challenge, even for further research, will be to define the role of geriatric preconditions and its impact on outcome. A second topic will be to clarify how, if ever, existing cardiological outcome values like mortality can be applied to elderly patients or if other values like improvement of functional status or quality of life are more suitable parameters to measure interventional success in elderly patients. The role of pharmacotherapy in advanced age has to be discussed. These topics will be embedded in epidemiological, pathophysiological and ethical aspects of interventional treatment of CAD in older age.

The renin angiotensin system is implicated in the progression of atherosclerotic disease as well as of left ventricular dysfunction. Angiotensin converting enzyme inhibitors and AT1 receptor antagonists have been proven to reduce morbidity and mortality in patients with left ventricular dysfunction or in those at high cardiovascular risk with preserved ventricular function. This book is intended to summarize evidences and provide a rationale for the appropriate use of RAS antagonists in cardiovascular diseases. It will be presented as highly practical information on this topic, written in a quick-access, no-nonsense format. The emphasis will be on a just-the-facts clinical approach, heavy on tabular material, light on dense prose. The involvement of the ISCP will ensure that the best quality contributors will be involved and establish a consistent approach to each topic in the series and this title is no exception. It will contain practical illustrations and is designed to improve understand and practical usage of cardiovascular drugs in specific clinical areas.

A comprehensive review the state-of-the-art in atherosclerosis of the arteries of the legs and feet. The authors discuss in detail the primary symptom-claudication-an intermittent pain in the leg or foot while walking, its predisposing factors, the current diagnostic methodologies, the impressive advances in the therapeutic armamentarium, and the need to screen for co-existing coronary artery disease. Additional chapters describe cutting-edge noninvasive angiography and vascular flow studies, specific drug therapy for claudication, regression of atherosclerosis therapy, gene therapy, and drug eluting stents for peripheral arterial disease. The authors also examine the epidemiology of LEAD, the effects of smoking and effective smoking cessation programs, its pathogenesis and its association with lipid abnormalities and hypertension, aggressive risk factor modification, and the need to measure the ankle brachial index of every patient over 45.
